RELEASE & AGREEMENT

I certify that I am initiating a consulting relationship with Jen Azevedo and for the provision of an equine evaluation and recommendations to optimize my horse’s health. I understand that Jen Azevedo is not a veterinarian, does not provide veterinary services, and that information derived from this consultation is not a substitute for veterinary services.

I hereby completely release Jen Azevedo and their agents, employees, assistants, independent contractors, co-sponsors, and affiliates, of any liability related to the provision of consultative services.

I acknowledge that I have been given the opportunity to ask questions regarding any aspect of this release, and by signing below, I acknowledge that I fully understand all aspects of the release and agree to these terms in their entirety. I hereby agree that should any legal proceeding arise relating to this general release and agreement, the disallowance of any term or condition will not prevent the legal enforcement of all other terms and conditions of this agreement.

I execute this release with the express intention of effecting the legal consequences, that is, the release of all liability associated with participating in equine consultative activities provided by Jen Azevedo, for myself, my executors, administrators, heirs, next of kin, successors, and assigns.
